Registration of Suppliers and Contractors for Year 2017

Applications are received by R.M.A. Rathnayaka, Secretary and Authorizing Officer Yatiyanthota Pradeshiya Sabha  upto 3.00 p.m. on 15th December 2016 for the registration of Suppliers and Contractors for Year 2017 for the supply of supplies and services shown in the schedule below. 1. Application should be submitted on self-prepared applications and the applications should be sent to the address given below to receive before the closing date and time. National Identity Card Number should be mentioned in the application.

2. For all supplies and services except for contracts a non-refundable fee of Rs. 1000.00 should be paid in cash or by a cheque or Money Order. Cheques and Money Orders should be drawn in favour of "Chairman, Pradeshiya Sabha, Yatiyanthota". The Money Orders should be payable at the Yatiyanthota Post Office.

For supplies, copy of the Business Registration Certificate should be sent with the application.

4. An assets certificate to the value equal to half the value applied for should be submitted when requested when registering for contracts.

5. Names of the suppliers who do not submit quotation when requested and of those who fail to supply when ordered and of those who fail to supply goods and services to the required standards will be removed from the list of registered suppliers and placed in the blacklist, without any prior notice.
